At the direction of the Director of Branch Operations, screens, evaluates, re-evaluates and treats patients referred by a licensed physician. Coordinates and communicates treatment activities with health care team members and the patient. Performance evaluations will be the responsibility of the Director of Branch Operations/designee.
RESPONSIBILITIES:
1. Evaluates the patient's need for physical therapy and develops appropriate treatment plan.
2. Carries out appropriate treatment program in accordance with agency policies and professional standards of care.
3. Instructs family members, PTA's, Home Health Aides, and other caregivers in appropriate physical therapy techniques.
4. Coordinates physical therapy progress with other caregivers in appropriate physical therapy techniques.
5. Refers patient to other professionals or agencies when appropriate.
6. Monitors and periodically re-evaluates patient's physical and emotional progress.
7. Prepares reports and maintains records in accordance with agency policy.
8. Communicates with the physician and other health care providers on a regular basis regarding the patient's progress.
9. Provides consultation and inservice education to staff on physical therapy procedures. Contributes to the professional development of new staff and health professional students.
10. Provides consultation and direct service to other individuals and groups in community, clinic, school, industrial and other settings in conjunction with the agency's program.
11. Supervises Physical Therapy Assistant. Makes joint initial visit with PTA to patient. Documents patient's conference with PTA every six PTA visits or every 30 days.
12. Seeks appropriate consultation from the Clinical Coordinator and agency Clinical Specialists and consultants.
Participates in case conferences, staff meetings, and inservice programs.
14. Participates in the evaluation of therapy services and agency programs and in the Evaluation of own job performance.
15. Keeps knowledgeable in homecare regulations.
Participate in the agency's continuous quality improvement program.
Meet agency's productivity expectations, census permitting.
GENERAL RESPONSIBILITIES:
Supports practices and communicates the Visiting Nurse Association and Hospice of
VT & NH?s Mission and Statement of Values.
Interacts with clients, referral sources, co-workers and others in a positive, consumer-
oriented manner.
Adheres to the Agency?s Organizational Ethics and Corporate Compliance Plan and
Safety Program.
Performs other duties as required or assigned.

1. Must have the ability to move intermittently throughout the workday.
2. Must have the ability to speak, write and read the English language in an understandable manner.
3. Must have the ability to plan, organize, develop, implement and interpret the programs, goals, objectives, policies and procedures of the services provided.
4. Must have the ability to cope with the mental, emotional and physical stress of the position.
5. Must be able to crouch, stoop, twist and reach above and below waist level.
6. Must have the ability to walk, lift, carry and transfer patients. Must be able to lift, push, pull or carry a minimum of 50 pounds and be able to move the weight a distance of five feet.
7. Must have adequate transportation.
8. Must have the ability to carry and use all assessment tools necessary for the proper evaluation and treatment of patients.
9. Must be able to navigate stairs of varying heights and degrees.
10. Must be able to maintain balance during patient transfers and ambulation.
11. Must possess adequate eyesight for visual evaluation of patients.
12. Must possess adequate manual finger dexterity to perform patient care and complete paperwork/computer work.
13. Must be able to tolerate variances in air quality, including but not limited to temperature variations, dust, smoke and animal hair.
